The ECOG Scale was developed by the Eastern Cooperative Oncology Group (thus the name) in 1955. The Eastern Cooperative Oncology Group was one of the first cooperative groups to perform multi-center clinical trials for the study of new forms of cancer treatment.
In order to better evaluate the effectiveness of treatments in clinical trials they created the ECOG Scale based primarily on freedom of activity of a patient post-treatment.
The scale is as follows:
| Grade | ECOG |
| 0 | Fully active, able to carry on all pre-disease performance without restriction. |
| 1 | Restricted in physically strenuous activity but ambulatory and able to carry out work of a light or sedentary nature, e.g., light house work, office work. |
| 2 | Ambulatory and capable of all self-care but unable to carry out any work activities. Up and about more than 50% of waking hours |
| 3 | Capable of only limited self-care, confined to bed or chair more than 50% of waking hours |
| 4 | Completely disabled. Cannot carry on any self-care. Totally confined to bed or chair |
| 5 | Dead |
ECOG is presently a worldwide group of more than 6,000 cancer researchers, doctors, and technicians which continues to engage in active research in the treatment of cancer. The ECOG Scale continues to be in active use and has remained an effective tool in determining the efficacy of cancer treatments for over 50 years.
