      [A tumor marker is a substance found in the blood, urine, or body tissues that can be elevated in cancer, among other tissue types. There are many different tumor markers, each indicative of a particular disease process, and they are used in oncology to help detect the presence of cancer.

      Not every tumor will cause an elevation in the tumor marker test, especially in the early stages of cancer. Physicians can use changes in tumor marker levels to follow the course of the disease, to measure the effect of treatment, and to check for recurrence.]

    • Findings of MRI, CAT, CT, bone scans, x-rays, etc.:
      • Positron emission tomography (PET): Is a nuclear medicine imaging technique which produces a three-dimensional image or map of functional processes in the body.
      • Magnetic resonance imaging (MRI): Is primarily a medical imaging technique most commonly used in Radiology to visualize the structure and function of the body.
      • Computed tomography (CT) or (CAT): Is a medical imaging method employing tomography. Digital geometry processing is used to generate a three-dimensional image of the inside of an object from a large series of two-dimensional X-ray images taken around a single axis of rotation.
  3. Any information you have regarding surgeries, chemotherapy hormone therapy or radiation.
  4. A list of all current medications, vitamins, minerals or any supplements
